By accessing or using MIZAN Health (“the Service”), you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use the Service. Your continued use of the Service constitutes acceptance of any updates to these terms.
2. Description of Service
MIZAN is a wellness tracking platform designed to support individuals working with healthcare providers to monitor nutrition, hydration, weight, and related health habits. The Service is intended as a wellness tool only. MIZAN is NOT a medical device and is not a substitute for professional medical advice, diagnosis, or treatment. Always consult your physician or qualified healthcare provider regarding any medical conditions or questions you may have.
3. User Accounts
MIZAN provides two types of accounts: clinic accounts for healthcare providers and patient accounts for individuals enrolled by a clinic. You are responsible for maintaining the confidentiality of your account credentials. Clinic accounts are responsible for their enrolled patients’ use of the Service. You agree to notify us immediately of any unauthorized use of your account.
4. Acceptable Use
You agree to use the Service only for its intended purpose of wellness tracking. You must not misuse the Service, attempt to gain unauthorized access, share your credentials with others, or use the Service to harm, harass, or deceive others. Clinics must only enroll patients who have given appropriate consent to be enrolled and to have their wellness data tracked.
5. Health Information Disclaimer
MIZAN provides wellness tracking and informational content only. Nothing on the Service constitutes medical advice. The platform does not diagnose medical conditions, prescribe treatments, or replace the judgment of a licensed healthcare professional. Users should always consult their physician before making changes to diet, exercise, or medication based on any information provided through the Service.
6. Data & Privacy
Your use of the Service is also governed by our Privacy Policy, which is incorporated into these Terms by reference. By using the Service, you consent to the collection and use of your information as described in the Privacy Policy. Please review it carefully at /privacy.
7. Subscription & Billing
MIZAN is offered on a monthly subscription basis for clinic accounts. Subscriptions renew automatically unless cancelled. You may cancel your subscription at any time. Cancellation takes effect at the end of the current billing period. We do not provide refunds for partial months. Pricing may change with reasonable notice.
8. Termination
We reserve the right to suspend or terminate accounts that violate these Terms, engage in abusive or fraudulent behavior, or misuse the platform in any way. We will make reasonable efforts to notify you before taking such action unless doing so would cause harm. Upon termination, your access to the Service will end and your data will be handled in accordance with our Privacy Policy.
9. Limitation of Liability
The Service is provided on an “as-is” basis without warranties of any kind. To the fullest extent permitted by law, MIZAN Health shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of or inability to use the Service. Our total liability for any claim shall not exceed the amount you paid to us in the three months preceding the claim.
